Thank you for selecting Special Needs Guidance Group for your benefits consulting needs. This letter sets out the terms of Our consulting services to You.
Services
We will coach You in navigating the Agency for Persons with Disabilities State of Florida (“APD”) application process. We have the experience, skills, and knowledge to help You with APD and are committed to providing our Services in a professional and workmanlike manner. We will assist you in matters such as:
- understanding how to complete the APD application
- identifying documents you need to collect
- developing an accurate and truthful narrative for APD to understand your circumstances and needs
- At your request, and if permitted by APD, we will make best efforts to attend any essential assessments and meetings you have with APD.
At Your request, We may agree to expand the Services to include additional tasks. This expansion must be agreed to in writing, such as an email exchange or signed letter.
While we cannot serve as Your representative or advocate before APD, we can teach you how to advocate for Your needs. We cannot guarantee any specific outcome including that Your application will be approved by APD.
Consulting Relationship
A consulting relationship is one where We provide the services defined in this letter at Your request. We are an independent contractor and not authorized as an employee, agent, or legal representative of You. We do not have the power to control Your activities and operations.
You are solely and exclusively responsible for the choices You make with respect to APD, including, but not limited to:
- Statements and Communications
- Documents
- Requests
- Decisions, Actions, and Inactions You make
Your choices and statements to APD should at all times be accurate and truthful. You must be authorized to act on behalf of the individual for whom you are submitting an application to APD.
Communications and Confidentiality
Communication is essential to performance of our Services. We each agree to communicate in a timely manner and acknowledge that extensive delays may impact the APD application. We will respect Your preferred mode of communication, be it in person meetings (if possible/available); telephone meetings; or email.
We understand and respect that the information you share with us is sensitive and private. We agree to keep it confidential, to not share it with others, and if appropriate, to destroy any documents you provide us. We may be required to disclose confidential information when it is required by law enforcement, a regulatory body, court order, or an applicable law, rule, or regulation.
Fees
The Fees for the Services is a one-time payment of $ 199.00. Fees are due and payable upon your acknowledgement of this Letter of Agreement. We cannot begin Services until we receive payment. Once Services have begun, the Fees are non-refundable.
If We agree to expand Our tasks, then the expanded tasks will include a statement of the additional fees.
Termination
Although it is Our goal to provide Services throughout the APD application process, there are times when either You or We may determine that the relationship between us must be terminated. If We make such determination, We will provide you at least thirty (30) days written notice and a pro-rata refund of your Fees. If You decide to terminate our relationship, Your termination will be effective two-days after you provide written notice to us. Upon Your request, we will return or destroy any documentation You provided to us. If You terminate, Your Fees are nonrefundable.
Governing Law
The terms of this Agreement are to be interpreted and construed in accordance with the laws of the State of Florida.
Any disputes regarding the terms of this Agreement shall be settled through arbitration in Duval County, Florida.

Your purchase demonstrates full understanding and agreement with the terms as set forth above.
